A member of this group, Christopher Burkey, has a product called OpenEdit which
fits your requirements almost exactly. I've used it as a wiki on several of my
engagements now and it works well. And it's open source to boot.

Please forgive me if this is slightly off-topic.
I'm looking for a wiki to use primarily for requirements definition, replacing
a pile of unmanageable Word documents and Excel spreadsheets. I've been
looking at using FitNesse, but it doesn't seem to offer what I'm looking for
without custom extensions (an option I'm still leaving open). I'll probably
still use FitNesse as well, just tying it to another port.
The features I'm looking for are the following:
* integration with CVS for versioning
* support for (pretty basic) templates (for things like use case realization)
* pdf export (or extensibility that will make this not too difficult via Apache
FOP (XSL-FO) or the like)
* ideally, Java-based and extensible (there's your obligatory on topic content)
