I second that recommendation -- good coverage of implementing a lightweight 
unified process as well.

The Reiterative Process

 

I found Applying UML and Patterns, by Craig Larman to be a good source for 
directly mapping business requirements to UML diagrams.  If you're not sure how 
to diagram something, this book includes different ways to develop your model.  
Especially if someone else needs to makes sense of it.
